Sammendrag
Bakgrunn:De hjemmeboende eldre er en sårbar gruppe grunnet aldersforandringer, funksjonssvikt ogkomorbiditet. Hjemmesykepleieren er ofte det eneste helsepersonell som har ansvar for åvurdere den eldre pasientens helsetilstand, og det kan være kritisk å oppdage forverring isomatisk tilstand tidlig slik at pasienten kan unngå akutt kritisk sykdom, komplikasjoner elleri verste fall - død.
Hensikt:Hensikten med denne oppgaven er å undersøke hva som gjør det mulig for sykepleier somjobber i hjemmetjenesten å oppdage en forverring i somatisk tilstand hos de hjemmeboendeeldre pasientene tidligere.
Metode:Bacheloroppgaven vår er litteraturbasert. Vi har benyttet en integrativ litteraturoversikt sommetode, hvor seks forskningsartikler ble analysert. Funn i disse artiklene, teori og egneerfaringer har dannet grunnlaget for å belyse problemstillingen.
Resultater:Funnene viste at det er en sammensatt prosess som involverer flere faktorer: grundigkartlegging, korrekte observasjoner og vurderinger, og organisatoriske aspekter innenforhjemmesykepleien. Background:Elderly people are a vulnerable group due to age related changes, functional impairment and comorbidity. The home nurses are often the only health professionals responsible for assessing an elderly patient's health, and it may be critical to detect deterioriation in the somatic condition so that the patient can avoid acute critical illness, complications or at worst - death.
Aim:The aim of this paper is to investigate what enables nurses who work in the home service to detect a deterioration in the somatic condition of the elderly patient earlier.
Method:Our bachelor thesis is literature-based. We have used an integrative literature review as a method, where six research articles were analyzed. Findings in these articles, theory and our own experiences have formed the basis for elucidating the problem.
Results:The findings showed that it is a complex process involving several factors: thorough mapping, correct observations and assessments, and organizational aspects of home nursing.
