Kommunal akutt døgneining i historisk lys

Abstract

Med samhandlingsreforma vart kommunane pålagt å opprette kommunal akutt døgneining (KAD). Eit hovudformål med denne nye institusjonen var å avlaste spesialisthelsetenesta. KAD-institusjonen er eit sengeposttilbod for pasientar med akutte allmennmedisinske problemstillingar. Han representerer eit omsorgsnivå i grensesnittet mellom spesialisthelsetenesta og primærhelsetenesta der kommunale lege- og omsorgstenester samhandlar med spesialisthelsetenesta om pasientane. Idéen om ei lokal teneste som skulle avlaste dei akutte døgntilboda i sjukehusa har spor langt bakover i tid. I artikkelen skildrar vi ut frå eit stiavhengigheitperspektiv korleis KAD-institusjonen vart danna som resultat av tre stiar som samverkar. Ein helsepolitisk sti handlar om korleis utvikling av lovverket opna for auka kommunalt ansvar for helsetenester. Ein annan sti handlar om gradvis aukande differensiering av dei kommunale helse- og omsorgstenestene. Ein tredje sti tar for seg korleis kjenneteikn ved tidlegare etablerte helseinstitusjonar har påverka og forma KAD-en som helseinstitusjon.

Municipal Emergency Unit (KAD) in a historical setting In the Coordination reform of 2012, the municipalities were required to establish municipal emergency care bed units (KAD). One of the main purposes of this new institution was to lessen the burden on the specialist health care services. The KAD institution is a municipal emergency care bed unit for acute admittance of patients with acute general medical conditions. It represents a level of care at the interface between specialist health care services and primary health care services where the municipality and the hospitals interact about the patients. The idea of a local service that relieves the hospitals of their acute 24-hour services has historical paths. The article describes and discusses, from a path dependency perspective, how historical imprints or paths have shaped the present KAD institution. Three paths are identified: the health policy path with a specific trajectory on legislation and an increased responsibility for health care services in the municipalities, the municipal services path which differentiate and enhance the number of health and care services, and the institutional path where former institutions set a specific trajectory of institutional development and consolidation of the current KAD-institution.
